Services
Our team provides a wide range of services for infants, children and adolescents including:

• Prenatal counseling;
• Newborn and infant care, including state of Texas-required screenings;
• Comprehensive well-child visits, including immunizations for all ages;
• Adolescent care;
• Treatment of acute and chronic illnesses;
• Routine lab work and screening;
• Developmental, hearing and vision screenings;
• Camp; school and sports physicals;
• Saturday morning appointments for urgent care only; and
• After hour call service .

At TCPA - Katy, our goals are to promote well being through preventive medicine and anticipatory guidance, provide appropriate treatment for acute illness and offer insight for optimal management of chronic disease.

Insurance accepted
Our office accepts many health insurance plans. To find out if your plan is on our approved-provider list, please visit the insurances accepted area or call our office.

Appointments
Appointments are required to see a physician. Our office hours are Monday through Friday 8 a.m. to noon and 1 p.m. to 5 p.m., Saturdays for urgent care only visits are from 8:30 a.m. to 11 a.m. by the on-call physician. Please call 281-398-3100 to schedule an appointment.

New patient paperwork
Please complete and send in the following forms, along with a copy of the front and back of your insurance card, to tcpakaty@texaschildrens.org or fax them to (281) 398-3145.

• Notice of privacy practices - explains the privacy rights of patient medical records
• Acknowledgement of privacy practices - indicates that the Notice of privacy practices was made available to you
• Patient financial policy - explains the policy for payment of services
• Texas Vaccines for Children (TVFC) patient eligibility screening - determines whether your child is eligible for TVFC. If your child is qualified for TVFC, please sign and date.
• TXDSHS immunization registry consent form - allow the State of Texas to have an electronic copy of your child's immunization record. If you consent, TXDSHS can replace your child's immunization records if they are lost. If you do not consent, please sign and date.

Additional information
Payments for each office visits may be made by cash, personal check or credit card. Our office personnel can help determine if there is a co-payment or deductible for a particular plan.

After an appointment, our physicians will write, call or fax in prescriptions. Our office may be called for routine prescription refills.

TCPA - Katy has newborn and admitting privileges at Christus St. Catherine Hospital, Memorial Hermann Katy Hospital and Memorial Hermann Memorial City Hospital.
